How do I make an appointment?

Walk ins are welcome during business hours. You can also call us at (801) 314-4188 during our working hours or you can request an appointment online by clicking "Schedule Appointment" below.

What is the average wait time if I walk in?

On a typical day, we’ll be able to see you almost immediately. That’s one of the best parts about our practice. We’ll get you in an get you back to doing what you love, fast.

What insurance companies do you accept?

We accept most insurance companies. To learn more about insurance and the companies we work with, please click HERE. Please feel free to call our office with any questions at (801) 314-4188 as we continue to add insurance partners.

Are you really more affordable than an ER or Urgent Care?

In almost every case, yes we are! Patients are billed for the cost of an office visit versus the expense of an emergency room or urgent care visit. If you have any additional questions about pricing, please give us a call.

What if I don’t have insurance?

We accept self-pay patients and have discounted rates. You will be asked to pay a $200 deposit at the time of your appointment.

Will my insurance cover my surgery?

Please contact your insurance by calling the number on the back of your insurance card to verify coverage. You can also contact our billing office with any questions that you may have.

When do I have to pay my balance?

After your insurance has processed your claim you will have 30 days to pay. We offer payment plans up to 12 months if needed.

What do I bring to my initial consolation?

Please bring the following to your consultation:

  • Driver’s license or a valid ID

  • Insurance information

  • Reports, x-rays, MRIs, CT scans, etc and any other relevant information

We encourage you to come to your initial consultation with a written list of questions to ensure you do not forget to ask them when you visit the doctor.

Are you a part of TOSH?

We are a private practice located at TOSH. If you have surgery, you will receive a bill from Intermountain Health Care and from us if visiting the Murray/TOSH location.

Are my medical records kept private and confidential?

Your medical file is handled with the utmost respect for your privacy. Our staff is bound by strict confidentiality requirements as a condition of employment regarding your medical records. We will not release the contents of your medical file without your consent.
