There are a few insurance companies that require a referral, please check with your insurance company. If you do not have insurance, we accept self-pay patients and have discounted rates - please contact our office for prices.

We ask that you contact your insurance company by calling the number on the back of your insurance card to verify that we are in network on your plan, some of our contracts are limited to specific plans and providers.
